Fill Out a Vietnam Visa Application Form (NA1) Without Mistakes

The NA1 form, known as the Entry and Exit Form from the Vietnam government, replaced the older M3 form and is required of all foreign passengers arriving in Vietnam by air who are collecting a visa on arrival with a visa approval letter. It can be completed in either English or Vietnamese.

This form is required for:

  • Travelers entering Vietnam by air with a visa approval letter to collect a visa on arrival
  • Travelers re-entering Vietnam after a visa run via land border who are collecting a visa stamp at the crossing

Where to get the NA1 Form

The NA1 form can be obtained in several ways, handed out on the airplane before landing, available at the VOA counter upon arrival at the airport, or can be downloaded in advance and filled out before the flight. 

Downloading and completing the form before your flight is strongly recommended. Arriving with a pre-filled form significantly reduces time spent at the VOA counter, particularly during peak hours at busy airports such as Tan Son Nhat (SGN) and Noi Bai (HAN).

How to fill out the Vietnam NA1 Form

The NA1 form has 20 numbered fields covering personal details, travel information, and visa preferences. Complete all fields in English or Vietnamese, both are accepted. Use CAPITAL LETTERS throughout for name fields.

‣ Field 1 – Full Name Write your complete name in CAPITAL LETTERS in the exact same order as it appears on your passport. Do not use nicknames or abbreviations.

‣ Field 2 & 3 – Sex and Date of Birth Mark your gender and enter your date of birth in DD/MM/YYYY format (e.g. 15/03/1988).

‣ Field 4 – Place of Birth Enter the city and country where you were born.

‣ Field 5 & 6 – Nationality Field 5 asks for your nationality at birth; Field 6 asks for your current nationality. These may differ if you have changed citizenship. Both should match the passport you are using for this trip.

‣ Field 7 & 8 – Religion and Occupation Both are optional. Enter your religion or write NONE. For occupation, enter your current job title or leave blank.

‣ Field 9 & 10 – Employer Address and Permanent Residential Address Field 9 is optional – enter your employer’s name and address if applicable. Field 10 requires your home address and contact number in your country of residence.

‣ Field 11 – Family Members Provide details of immediate family members – parents, spouse, children, and siblings – including their full name, sex, date of birth, nationality, and home address. This is for immigration reference only and has no impact on your entry.

‣ Field 12 – Passport Details Enter your passport number, specify the type (Ordinary, Official, or Diplomatic), the issuing authority, and the expiry date.

‣ Field 13 – Previous Entry into Vietnam Enter the date of your last visit to Vietnam. Leave blank if this is your first time.

‣ Field 14 – Intended Entry Date and Length of Stay Enter your expected arrival date in DD/MM/YYYY format and the number of days you plan to stay. The arrival date must not be earlier than the date stated on your visa approval letter, and the total stay must not exceed the exit date on the same document.

‣ Field 15 – Purpose of Entry Enter the purpose exactly as stated on your visa approval letter – Tourism or Business. Do not deviate from the wording on your letter.

‣ Field 16 – Temporary Residential Address in Vietnam Provide the full address of where you will be staying – hotel name, street, district, and city. Use the exact address from your booking confirmation.

‣ Field 17 – Hosting Organization or Individual If entering on a business visa, enter the name and address of the sponsoring company. For tourist visa holders, write N/A unless staying with a local host, in which case provide their name, address, and relationship to you.

‣ Field 18 – Accompanying Children Under 14 If any child under 14 is traveling on the same passport, enter their full name, sex, and date of birth, and attach their photo. Leave blank if not applicable or if the child holds a separate passport.

‣ Field 19 – Visa Type and Validity Select Single or Multiple entry and enter the validity dates exactly as shown on your visa approval letter.

‣ Field 20 – Other Requests Enter any special requests related to your visa application, or write N/A.

Signature section: Enter your port of arrival (Hanoi, Ho Chi Minh City, Hai Phong, Da Nang, Nha Trang, Can Tho, Phu Quoc, or Da Lat) and the date of arrival. Sign and print your full name in the applicant signature box.

Photo attach one passport-sized photo (4×6 cm, white background, front-facing, no sunglasses or headwear) to the designated space. A second photo must be submitted separately alongside the form at the VOA counter.

Sometimes, minor errors on the NA1 form are enough to cause delays or complications at the VOA counter. Make sure you have :

  • Writing your name in CAPITAL LETTERS
  • Entering dates in the right format, always use DD/MM/YYYY
  • Not listing an entry date earlier than the date on your approval letter
  • Providing an accommodation address that match your booking
  • Not forgetting to attach the photo

What to do after completing the form

The NA1 form must be presented to the immigration officer upon arriving at the airport in Vietnam. For visa on arrival travelers, it is submitted at the Landing Visa / VOA counter in the international arrivals hall together with:

  • Printed visa approval letter
  • Original passport
  • 2 passport-sized photos (4×6 cm)
  • Stamping fee in cash (USD 25 for single entry / USD 50 for multiple entry)

Once the officer processes your documents and affixes the visa sticker to your passport, you proceed to the standard immigration counter for your entry stamp.

Tip: Always complete and print the NA1 form before boarding your flight. Filling it out at the airport counter adds unnecessary time to an already lengthy process.
